High Reliability Videos


Saint Mary's Hospital has joined the Connecticut Hospital Association (CHA), as well as hospitals across Connecticut, in the mission to eliminate preventable errors and harm events by adopting the "Safety Starts with Me" patient safety culture.

The following videos and materials will discuss the CHAMP toolkit – essential methods used to create the safest possible conditions within our health system. Completing this course is a requirement for all Saint Mary's staff, volunteers and medical staff.

What to do:

  • Watch each of the seven videos in its entirety.
  • Complete the exam. At the end of the seventh video, click the link provided to access the exam (enter your first and last name, and use the access code in lowercase safety).
  • A score of 80% or higher is required to pass the exam.
