Johnson Memorial Hospital, Inc. (The Applicant) is filing a Certificate of Need pursuant to Section 19a-638 of the Connecticut General Statutes. The Applicant is proposing to convert the surgery center at 148 Hazard Avenue, Enfield, CT, from a hospital-based outpatient department to a freestanding outpatient surgical facility located adjacent to the existing site. The Applicant will have a 51% ownership of this facility.
