Is Mary’s Meadow right for me?
If you or a loved one needs short-term rehabilitation services or long-term skilled nursing care, Mary’s Meadow is the ideal setting.
- Short-term rehabilitation: Restore function and strength so you can regain your highest possible level of mobility and independence, and resume living at home.
- Long-term skilled nursing care: If you cannot live safely and independently at home, and require assistance with the activities of daily life, a long-term skilled nursing facility serves as a permanent residence with around-the-clock nursing care to meet your physical, social and psychological needs.
Transferring to Mary’s Meadow
If you and your doctor have determined that short-term rehabilitation is right for you and you are preparing for surgery, you, your doctor or your case manager may contact our admissions office at 413-531-0532. A representative from Mary’s Meadow will assist you with a pre-admission screening to plan your stay or coordinate your transfer.
If you wish to become a long-term resident at Mary’s Meadow, please contact our admissions office at 413-531-0532. You will be asked to complete an application. Our admissions coordinator is available to answer questions you may have.
What to expect
- Short-term rehabilitation: Your course of treatment and length of stay will be tailored to meet your needs, abilities and personal goals. You’ll be under the care of an interdisciplinary team including skilled therapists who’ll work closely to help you achieve your personal best. While each care plan is different, your daily schedule will likely include:
- Morning and afternoon therapy sessions, with time for rest if needed.
- Breakfast, lunch and dinner meals planned by a nutritionist to meet your current medical needs.
- Nursing staff will help you with dressing and grooming in the morning, and getting ready for bed in the evening.
- Nursing staff will provide needed medical treatments during your stay.
- Throughout your stay, you’ll receive routine visits from our Medical Director who specializes in rehabilitation.
- Your Mary’s Meadow Case Manager/Social Services – who coordinates your care – will regularly visit during your stay and is always just a phone call away if you or family members have questions about your care.
- Long-term skilled nursing care: Living at Mary’s Meadow provides around the clock medical care.
- You’ll be settled into your own, comfortably furnished room with its private full bathroom. We encourage you to bring personal items such as family photos, knick knacks, comfortable throw or artwork, books and music, in addition to your clothes and toiletries.
- You’ll quickly get to know the nurses and Elder Assistants who will be caring for you.
- Your Elder Assistant will ask you or your family about your personal preferences when it comes to your care and what activities you’d like to be involved in, such as, when you’d like to eat, and what you like to do each day.
- Within two weeks, you’ll have what’s called a care plan meeting where you’ll interact with the staff directly involved in your care and treatments. Your family members are encouraged to attend these meetings, which subsequently are held quarterly.
- You can take part in social activities in your home’s common areas, and go on Mary’s Meadow-sponsored outings to area attractions. If you’re physically able, you can also leave Mary’s Meadow to go out to eat or for a longer outing with family or friends.
- Staff is always available to help you with your daily self-care activities such as bathing and dressing.
- Above all, the staff at Mary’s Meadow are committed to ensuring that you feel comfortable, safe and happy in your new home.
Insurance and financial information
Rehabilitation is covered in full or in part by most health insurance plans, including Medicare. Mary’s Meadow accepts most major insurance plans.
Long-term skilled nursing care may be paid Medicaid, Long Term Care insurance or private funds, depending on your individual needs and financial situation. Please contact our admissions office at 413-531-0532 to discuss your particular situation.
