These accomplishments recognize hospitals that maintain high standards for treatment of heart failure patients. The heart failure program also consistently receives the Get With The Guidelines - Heart Failure Gold Plus Quality Achievement Award.
We offer both inpatient and outpatient facilities to provide expert, innovative and organized care.
Trinity Health Of New England is home to the first dedicated inpatient and outpatient heart failure program in Connecticut. The inpatient unit is designed to provide highly specialized, quality care to patients hospitalized with heart failure. After hospital discharge, the team will coordinate care with our outpatient clinic, The Center for Cardiac Recovery & Advanced Heart Failure. The multidisciplinary team of healthcare professionals includes:
- Board certified heart failure cardiologists
- Heart failure trained Advanced Practice Providers
- Heart failure trained registered nurses
- Heart failure registered nurse navigator
- Social worker
- Registered nurse case manager
Additional services that are offered with our heart failure program include:
- Education - we offer dietary counseling, medication management, lifestyle coaching, exercise recommendations and much more.
- Support - social workers and nurses who are dedicated to heart failure management can assist patients with care coordination, counseling, and emotional support. We also offer heart failure patient support groups.
- Home care- we help coordinate heart failure home care, which may include remote monitoring, home assessments and home care provider referrals.
- Palliative care- our palliative care specialists partner with the heart failure team to guide patients and caregivers as they navigate the spectrum of heart failure. The palliative care team assists patients and families with psychosocial support, symptom management, integration of medical, psychological and spiritual aspects of care.
The Center for Cardiac Recovery and Advanced Heart Failure outpatient clinic is the first dedicated inpatient/outpatient heart failure program in Connecticut. It includes a multidisciplinary team of healthcare professionals who provide innovative and expert medical care to adult patients with acute and end-stage heart failure. Patients receive specialized treatment and support from a team who is completely focused on heart failure, whether in the hospital or at home. The heart failure team consists of:
