The Hoffman Heart LifeCare Program is a multidisciplinary outpatient service that helps patients better manage their health and well-being to reduce their risk for heart disease. Our team will work with you to help create a personalized plan of action. You will meet with a registered dietitian and exercise physiologist to discuss nutrition and exercise goals to meet your individual needs.
