- Together for Wellness Services: Together for Wellness is a comprehensive program designed to help you take control of your health combining nutrition, clinical support, and community resources to support lasting lifestyle changes.
Community Based Programs
- Diabetes Prevention Program: This program is designed for people with prediabetes to cut their risk of developing type 2 diabetes through lifestyle changes such as healthy eating and physical activity.
- Medical Home Program: This program focuses on building strong partnerships between families and their pediatric care providers.
- Putting on AIRs Program: This program compliments the Medical Home Program by providing in-home asthma education and management.
- Connecticut Early Detection and Prevention Program (CEDPP): This program empowers women with the knowledge and tools to reduce their risk for heart disease and stroke
