Trinity Health Of New England has transitioned to a new patient portal – Trinity Health MyChart – to better meet your needs. Any new information from visits to a Trinity Health Of New England doctor’s office or hospital from November 2, 2024 and beyond, including prescription refills, visit notes and lab results, will now be in your new MyChart patient portal.
Some information prior to November 2, 2024 may not be available in your new account due to differences in the versions.
You will continue to be able to view your old data in your former MyCare/MyChart account. Your providers will always have access to your historical medical data.

You can still access your historical medical and billing information for visits prior to November 2, 2024 from our previous patient portals, MyCare and MyHealth. However, you will only be able to view and read the information; you will not be able to communicate with your provider and no new information will be added.
