Online patient portals offer patients personalized and secure on-line access to portions of their medical records. It enables you to securely use the Internet to help manage and receive information about your health. 

As we continue to care for you during the COVID-19 pandemic, our telephone lines might be busier than usual. You are encouraged you utilize our secure patient portal, which allows you and your provider to communicate directly. You can also use our CareConnector virtual assistant to help with:

  • Prescription refill requests
  • Non-urgent medical questions or concerns
  • Scheduling non-urgent appointments
  • Questions about recent laboratory or radiology results
  • Referral requests

New Changes to MyCare and MyChart

To engage our patients even more in their care and to comply with the 21st Century Cures Act, we will share most of the information in your health record to the MyCare/MyChart* patient portal immediately after it is finalized. This includes releasing most test and lab results immediately, as well as sharing your provider’s visit notes with you.

What is MyCare?

Thanks to secure technology, it is possible to view health summaries from the MyCare electronic health record, access most test results, and review trusted health information resources. Whether you are looking to manage a medical condition, review test results or pay your bill, MyCare is the tool for you. We know how busy life can be and so we are pleased to offer a mobile version of MyCare available in the Google Marketplace or Apple Store.

How to sign up for MyCare

Trinity Health Of New England patients are encouraged to register for MyCare, a service providing free, personalized, and secure online access to portions of their medical records. This service allows patients to safely receive and manage information about their health.

You will be issued a MyCare activation code on your After Visit Summary print-out at the end of your hospital stay. This code will enable you to log in and create your own username and password on the MyCare website.

MyCare will allow you to pay multiple provider bills. Please use your activation code from your statement.

How do I connect my medical record to an app?

To access your record through one of these apps (or someone else's where you have proxy rights) you must first create a Trinity Health Of New England patient portal account.  If you do not have access, please sign up by creating a Trinity Health Of New England account.
