Community Health

Community Health and Well Being (CHWB) is a key pillar of Trinity Health of New England's integrated people-centered health system. This calls for addressing physical, spiritual, and behavioral health needs of people who live in our communities throughout Connecticut and Western Massachusetts. Issues such as lack transportation, housing, food security, safety and education, or language and socio-economic barriers influence health outcomes. In the United States, health care spend is almost entirely focused on clinical care, not preventative or public health. However, clinical care and genetics account for only 30 percent of the factors that impact the health of populations. Trinity Health of New England serves as a people-centered health system addressing all influencers of health, not just episodic clinical health care. Through programming, referrals, and service coordination, our team strives to build individual and community self-sufficiency and well-being.
