Learner Onboarding
We are very excited for you to start your experience at Trinity Health Of New England.
These are links and instructions for our onboarding process at Saint Francis Hospital and Medical Center and Mount Sinai Rehabilitation Hospital.
If this is your first time here, please expand the accordion below to complete the onboarding process for a New Learner.
If you have had previous educational experiences here and completed the onboarding process, please expand the accordion below to complete the onboarding process for a Returning Learner.
Step 1:
Confirm with your preceptor/coordinator that your educational institution has an active affiliation agreement* with Trinity Health Of New England.
*You cannot proceed further if your educational institution does not have an active affiliation agreement with Trinity Health Of New England.
Step 2:
Download and submit:
Have the Health Certification Form completed by your physician or school’s health center at least 4 weeks prior to your start date. Send the completed form to Occupational Health via email at OccHealth@TrinityHealthOfNE.org.
Please note: A Hepatitis B Waiver Form is required if learner has not received the vaccination series. Submit completed Waiver Form along with your Health Certification Form to Occupational Health.
Retain a copy of your Health Certification Form. If your rotation occurs during the flu season you will need to take your Health Certification Form with verification of your flu vaccine to one of the flu clinics where you will be given a flu validation badge.

Read the materials and view the presentation below:

Step 4:
Please complete and electronically sign each of the seven (7) Onboarding Forms and click the “Submit Completed Forms” button at the bottom of the last page.
- If your device does not allow you to submit the Onboarding Forms, please save the completed forms to your computer and email them as an attachment to learners@TrinityHealthOfNE.org.
Step 5:
Please consult with your preceptor/coordinator regarding badging, which can be done one of two ways:
1. Your preceptor/coordinator will ask you to submit a Badge Form for them to sign.
- Complete and send it to your preceptor/coordinator, they will sign and return it to you.
- Bring the form to the Badging Office during badging hours to have your photo taken and badge.
Badging hours are as follows:
- Saint Francis Campus
Mondays & Thursdays | 7:30 - 11:30 am
Wednesdays | 1:00 pm - 5:00 pm
Fridays | 11:00 am - 3:00 pm - Mount Sinai Campus
Tuesdays | 7:30 - 11:30 am
2. Your preceptor/coordinator will ask you for a picture for your badge.
- Send your preceptor/coordinator a JPEG of yourself in front of a white background.
- They will then arrange for your badge and provide it to you on your first day.
Step 6:
Parking tags can only be issued once you have received an ID badge.

Complete the form and submit it to your preceptor/coordinator who will arrange for your parking assignment and will provide you with your parking tag on your first day. Confirm with your preceptor/coordinator where you should park on your first day.
On your first day:
- Please ask your preceptor/coordinator any questions you have about any of the materials.
- Your preceptor/coordinator will provide you with your ID badge and parking tag or instruct you on where to pick these up.

Step 2: Please only complete if your rotation is during the flu season.
Download and submit:

Please complete and electronically sign the two (2) Onboarding Forms, then click the "Submit All Forms" button at the bottom of the second page.
Step 4:
Please provide your badge number to your preceptor/coordinator and they will confirm that it is still active.
Step 5:
Parking tags can only be issued once you have received an ID badge.
Your preceptor/coordinator will confirm that your parking tag is active or will reactivate it if it is not. Please notify them if any vehicle information has changed, such as the type of car or the license plate number.

On your first day:
- Please ask your preceptor/coordinator any questions you have about any of the materials.
- Your preceptor/coordinator will provide you with your updated badge or parking tag if applicable.
