For Our Office Patients:

You will be asked to have your temperature taken. If your temperature is elevated, you will be evaluated by a member of our Clinical staff before proceeding with your visit.

You will be asked to wear a mask when you enter our office. We can provide one if needed. IF YOU REFUSE TO DO SO, YOUR VISIT WILL BE RESCHEDULED.

We are making every effort to ensure appropriate social distancing requirements. Our waiting areas have designated seating to accomplish this.

In the event we have an excess of patients at any given time, and we cannot meet the social distancing requirements, you may be asked either to wait in another area of our office, or in your car. A member of our staff will advise you when your provider is available.

Unless absolutely necessary, PLEASE COME TO YOUR APPOINTMENT ALONE. Plainly spoken, we simply do not have enough room in either our waiting area, common areas or exam rooms to accommodate extra people and maintain safety and social distancing requirements.

If possible, please bring a copy of your insurance card with you – front and backsides.
