How do I become a patient of Pioneer Valley Cardiology?
Our cardiology practice requires a referral from your Primary Care Physician (PCP). Physician offices may contact us at our main number, (413) 748-7095 Option 1, to speak to an Access Services Representative. Additionally, referrals may be faxed to our office to (413) 732-1214.
As a new patient to our practice, it is important that we have all necessary records, consents, and medical history ready for your first appointment. This information helps your cardiologist understand your medical needs and history.
Initially, you will be scheduled with one of our Board Certified Cardiologists. Follow-up care will be shared with your cardiologist's Advanced Practice Provider. PVCA uses a Care Team model of care to ensure that you always have access to your providers. Care Teams consist of your cardiologist, an Advanced Practice Provider (APP), and medical assistant. Your cardiologist will always work closely with their APP and review every appointment scheduled with him/her.
Please bring the following to your first appointment:
- Authorization for Release of Medical Records
- Patient Demographics & Medical History
- Consent for Treatment
- Release of Information
- Current Medication List
- Photo ID
- Insurance Card(s)
- Copay

We ask that you bring your registration forms with you to your appointment. If you have any questions regarding our New Patient packet, please feel free to contact our Patient Access Team at (413) 748-7095 Option 1. These forms may also be confidentially faxed to (413) 732-1214.
Your medical history and medication list are vital pieces of information that are necessary for a successful first visit. PVCA will work hard to gather as much of this information that we can; if you have records or pertinent information that you feel we should know, please bring this with you to your first appointment.
Patients are responsible for verifying with their insurance company whether an insurance referral is required prior to this initial appointment. If this appointment is no longer needed, please notify your Primary Care Physician or PVCA within 24 hours of your scheduled appointment.
