Announcing Our New Care Teams 

Care Teams are becoming more and more the standard of care in specialty practices across the country. Care Teams include your primary cardiologist, an Advanced Practice Provider, and Medical Assistant. The old adage, "two sets of eyes are better than one" might come to mind. Pioneer Valley Cardiology has implemented this care model and the results have been excellent. Our patients have improved access to their cardiologists, especially when it really matters. Your cardiologist has hand-picked their partner in caring for you. The APP's at Pioneer Valley Cardiology are highly skilled, cardiac-trained, clinicians who work in tandem with your assigned cardiologist. After every appointment, your care and visit is reviewed by your MD and discussed within your care team. This approach ensures that you are receiving the excellent care you're accustomed to. 

Patients will see their APP for routine follow-ups after a hospitalization, for triage visits, and as part of your routine care. Pioneer Valley Cardiology will ensure that you see your cardiologist at least once per year.
