General FAQs 

Why was the decision made for THOfNE to exit SoNE HEALTH? 
Over the last several months, multiple options were evaluated by SoNE HEALTH’s Board of Directors to ensure financial viability and future success of the network. Ultimately, independent providers of the board made the recommendation for SoNE HEALTH to evolve into an independent provider network. As a result, the decision was made for TH Of NE and its hospitals and employed providers to exit SoNE HEALTH. This change will be effective January 1, 2024.

What is the effective date of this transition? 
The effective date of these changes is January 1, 2024. 

What impact does THOfNE’s decision have on my relationship with SoNE HEALTH? 
This change will not impact affiliated providers as SoNE HEALTH will continue to provide its current services to those members in the Clinically Integrated Network (CIN). However, we encourage you to have these discussions with SoNE HEALTH leadership if you have any concerns. 

What impact does this have on THOfNE's current commercial payor negotiations, like Cigna and Anthem? 
For affiliated providers who remain with SoNE Health, this change will have no impact on commercial contracts and SoNE Health will continue to negotiate on your behalf.

Will Trinity Health Of New England join a new CIN? 
THOfNE has established a new CIN and Medicare ACO, Trinity Health Of New England CIN, LLC (THOfNE CIN), effective January 1, 2024.

For calendar year 2024, the THOfNE CIN is specific to the Medicare Shared Savings Program (MSSP) ACO and Medicare beneficiaries of the THOfNE Medical Group.

The THOfNE CIN will also explore opportunities for its integrated network to engage in commercial contracting but does not anticipate having all commercial contracts in place for affiliated physicians by January 1, 2024. 

Will I have the opportunity to join THOfNE’s CIN? 
Yes, participants in the CIN can include both employed and affiliated providers, with affiliated providers being able to join as early as January 1, 2025. 

What if I am interested in joining the THOfNE CIN’s ACO? 
If you are interested in joining THOfNE CIN’s ACO:

  • Please fill out this form noting your interest
  • A participation agreement will need to be signed and returned to us by early summer 2024 for an effective date of 1/1/2025
  • You will also need to terminate your existing Medicare ACO agreement and be removed from that agreement by August 2024
  • We will continue to provide detailed information about participation and respective deadlines

Patient Care FAQs 

What impact will this have on our patients? 
There will be no disruption to clinical care or clinical partnerships. THOfNE and SoNE HEALTH are committed to an amicable separation and to keeping the best interest of our patients and the community at the center of this work. THOfNE hospitals and providers will continue our commitment to delivering high-quality care at the lowest cost and to partner with our affiliated physicians.

Is my Epic Community Connect contract impacted by this change? 
The Epic contract is between THOfNE and you/your practice. THOfNE is committed to supporting practices currently on Epic and those transitioning to TogetherCare. There will be no changes to the current arrangements as a result of the THOfNE and SoNE HEALTH separation.

How will this change impact Colleague Health Plan tiering? 
All providers who participate in the THOfNE CIN, will continue to be Tier 1 for CY2024. We will be working with SoNE HEALTH and other aligned providers to provide Tier 1 continuity for our colleagues who seek care from these providers today and to address the needs of our colleagues.


Community Connect FAQs 

Is the go live date for TogetherCare May 4, 2024, for all practices on Epic? 
Yes, all of THOfNE practices and facilities, as well as all Community Connect practices, will go live on TogetherCare on May 4, 2024.

For those Community Connect Practices that signed TogetherCare EMR Participation and Use Agreements, are there any changes other than the implementation timeline that will be made to the contract? 
The only terms changing in those contracts is moving the Effective Date to 05/4/24.

In terms of the subsidy, what is the amount of that subsidy? Has that changed from last year? Are there additional criteria that is required of the practices to get the subsidy?There will be no changes in methodology for determining the subsidy for the annual and one-time costs to the practices for TogetherCare. Annual costs will continue to be based on the size of the practice (users, providers, visits, etc.). Subsidy for annual costs will be based on the Tier Structure.

Community Connect providers will continue to be eligible, as long as they are a medical staff member at one of the THOfNE hospitals.

Will THOfNE contact the practice directly about the TogetherCare implementation, as well as any contract requirements? 
The TogetherCare contracts are between THOfNE and the individual practices. SoNE HEALTH is not a party to current Epic or future TogetherCare contracts. THOfNE will work directly with practices throughout this implementation.

Is there one point of contact for Community Connect TogetherCare Implementation? 
Patrick Gibbons is the primary point of contact for the Connect TogetherCare implementation activities and any legacy Epic issues. Post implementation there will be a dedicated resource to support the Connect practices.
