Healthy aging has become more important than ever as older adults lead active lives longer.

Factors that influence healthy aging may be uncontrollable and based upon genetics, while others can be proactively managed, according to the National Institute on Aging (NIA). Regular checkups and annual screenings catch disease early when it’s most treatable. They also support your goal to live an active and independent lifestyle.

Perhaps you already understand the importance of annual screenings and take the time to get them done. Wouldn’t it be nice to be rewarded for your effort?

Our plans ensure you receive annual screenings critical to healthy aging without concern over cost. Better yet, we reward you for completing eligible screenings through our Health Plan Member Rewards program.

All members can receive either an Annual Wellness Visit or In-Home Assessment to earn a member reward.1 Qualifying members can also earn rewards for completing eligibility-based screenings such as breast cancer screening, colorectal cancer screening, diabetes care eye exam, and diabetes care A1c. Members earn a $25 reward loaded on their Flexible Benefit Card (new for 2024) after completing the necessary attestations.

Making the most of our member rewards once you’re enrolled

Call Member Services at 1-800-240-3851 (TTY 711) between 8 a.m. and 8 p.m.

Members are notified by mail throughout the year to inform them when they are eligible for eligible screenings. 

Participating in the Member Rewards program is completely optional; if you do not want to receive a reward for your qualifying screenings, you don't have to do anything at all!

Members can report a reward activity by completing and returning the attestation form included with your notification. The form should include the service received, the name of your provider, and the date of your visit. Members can also call our Member Services team to complete a verbal attestation.

Rewards typically arrive within 30 days of receipt of the member's attestation.

Your Flexible Benefit Card (new for 2024) will also hold your Member Rewards once earned. Amounts will automatically be loaded to your Flexible Benefit Card and can be used at most retailers that accept Visa. 

Please note your Member Rewards benefit may not be used on alcohol, tobacco or firearms.
