Social Services and Spiritual Care

Social Services
Licensed social workers provide counseling and support to patients and families and can provide information on a variety of community, public and private services and programs. These might include financial assistance, crisis intervention, mental health services, child or adult protective services and substance abuse programs.

Should a patient require admission to an extended care facility, a team of specially trained social workers will assist by arranging for admission and transfer to a sub-acute facility, nursing home, rehabilitation center or rest home.

Spiritual Care
The Spirit is a natural dimension of each one of us. Spirituality describes an understanding and awareness of relationships, an appreciation of a life purpose and a capacity to make meaning with regard to persons or events experienced in life. For some people it is their religion that helps them cope with illness, trauma, loss, and even death; for others it is their personal spirituality. As clinical members of a holistic healthcare team, chaplains play a vital role by journeying with people searching to make meaning of such life events.

Our chaplains demonstrate their dedication to human dignity through their deep respect for the uniqueness of each individual’s spiritual journey.

Chaplains are available to:

    • Provide spiritual and emotional support for patients, families and colleagues
    • Provide assistance in understanding advance care directives
    • Facilitate alternative modes of healing
    • Facilitate family meetings to encourage understanding of the plan of care
    • Participate in a continuum of spiritual care that emphasizes connection to local priests, ministers, rabbis, imams or other spiritual advisors or faith groups
    • Offer the sacraments: Reconciliation, Eucharist, and the Sacrament of the Anointing of the Sick
    • Provide counsel regarding ethical issues or concerns
